It helps adults at high risk of developing type 2 diabetes to reduce their risk of developing the disease by taking steps that will improve their overall health and well-being. The Care Coordinators conduct a lifestyle change curriculum to reduce weight and increase physical activity by improving the quality of life of people with pre-diabetes.
Diabetes Self-Management Education Program (DSMEP)
New classes starting soon! Dates to be announced
8 group sessions for uninsured persons and 8 one-to-one sessions for those with insurance. A free glucose meter is provided to every participant who completes the course, consisting of a two-hour weekly class for eight weeks.
Support Groups
They are designed to provide an ongoing process after the 8 Diabetes classes are completed. The 2-hours group sessions create a safe environment for participants to learn how to take control of their health by engaging in discussions on health such as diabetes, hypertension, pre-diabetes, and obesity management.
Maternal Health Support Group
Supporting pregnant women and new mothers in the following topics: Pregnancy, Preeclampsia, Gestational Diabetes (free glucometer), Post-natal care, Post-partum depression, and Breastfeeding. If you’re planning your pregnancy or have a newborn to 12-month-old baby and have been diagnosed with hypertension we will bring you education support and a free blood pressure monitor.
Zumba Classes
A trained instructor leads Zumba classes on Tuesdays and Thursdays from 9:00 am to 10:00 am at 4866 Hercules, Linda Vista Apartments Community Room. Cost per class: $2 (or $1 if participants also attend our DPP or DSMEP classes or if a Linda Vista Apartment resident)
