Diabetes Self-Management Education Program

Through education and awareness, we provide the necessary tools for adequate diabetes self-management and the prevention of complications. The Diabetes Self-Management Program helps improve the quality of life for people with diabetes and those at risk of developing the disease.  We offer 7 group sessions to participants without insurance and 7 one-on-one sessions for those with insurance.

A free glucometer is provided to every participant completing the whole course, which consists of a two-hour weekly class for a period of 7 weeks. Learn about the following topics:

  • Understanding the Human Body 
  • Types of Diabetes and Understanding Carbohydrates
  • Food Groups and Reading Labels
  • Self-Monitoring
  • The Importance of Physical Activity 
  • Diabetes Complications
  • Medications

Graduates of the Diabetes Self-Management Education program hold up their certificates of completion.
