Pre-Diabetes Care
Pre-Diabetes Care supports the retention and continuum of care of patients at risk of developing a chronic disease, such as Diabetes, by providing care follow-up, education, and referrals to needed social services, in order to reduce patients’ health disparities and address their non-medical drivers of health.
The Diabetes Prevention Program (DPP)
The Diabetes Prevention Program (DPP) helps adults at high risk of developing type 2 diabetes to reduce their risk for developing the disease by taking steps that will improve their overall health and well-being. The Care Coordinators trained as a life style coaches conduct a lifestyle change curriculum through a psychosocial approach to reduce weight and increase physical activity by improving the quality of life of people with pre-diabetes. We offer one-year program in group or one to one setting to individuals with:
- Pre-Diabetes Diagnosis
- Fasting glucose between 100-125 mg/dl
- Pre-Diabetes in risk assessment
- A1C of 5.7 to 6.4
- BMI >25 and 23 if Asian
Zumba classes
- Tuesday and Thursday 9:00 - 10:00 AM at Linda Vista Apartments, 4866 Hercules Ave, El Paso, TX 79904. Cost $2 dlls per class
Healthy Cooking Recipes
The program changed in person classes by healthy cooking recipes videos through Facebook: Bueno para Comer. This platform allows to connect with a virtual community interested in Chronic Care and Prevention Strategies services and schedules for virtual health education classes conducted by Chronic Care Coordinators.
