_{The Diabetes Prevention Program (DPP)}

The Diabetes Prevention Program (DPP) helps adults at high risk of developing type 2 diabetes to reduce their risk for developing the disease by taking steps that will improve their overall health and well-being. The Care Coordinators trained as a life style coaches conduct a lifestyle change curriculum through a psychosocial approach to reduce weight and increase physical activity by improving the quality of life of people with pre-diabetes. We offer one-year program in group or one to one setting to individuals with:

• Pre-Diabetes Diagnosis
• Fasting glucose between 100-125 mg/dl
• Pre-Diabetes in risk assessment
• A1C of 5.7 to 6.4
• BMI >25 and 23 if Asian

• Tuesday and Thursday 9:00 - 10:00 AM at Linda Vista Apartments, 4866 Hercules Ave, El Paso, TX 79904. Cost $2 dlls per class

_{Healthy Cooking Recipes}

The program changed in person classes by healthy cooking recipes videos through Facebook: Bueno para Comer. This platform allows to connect with a virtual community interested in Chronic Care and Prevention Strategies services and schedules for virtual health education classes conducted by Chronic Care Coordinators.
