Diabetes Self-Management Education Program

The Diabetes Self-Management Education Program at Project Vida is funded and accredited by the American Association of Diabetes Educators. It was one of the first community classes offered by Project Vida, becoming formalized 12 years ago after a nurse would go to community centers to teach people about the risk for diabetes, a prevalent issue in El Paso. Now, the class has a 7 class curriculum, which is taught by a chronic care coordinator, and anyone who finishes the class receives a free glucometer.

About the Class

Through education and awareness, we provide the necessary tools for adequate diabetes self-management and the prevention of complications. The Diabetes Self-Management Program helps improve the quality of life for people with diabetes and those at risk of developing the disease. We offer 7 group sessions to participants without insurance and 7 one-on-one sessions for those with insurance.

A free glucometer is provided to every participant completing the whole course, which consists of a two-hour weekly class for a period of 7 weeks. Learn about the following topics:

  1. Understanding the Human Body

  2. Types of Diabetes and Understanding Carbohydrates

  3. Food Groups and Reading Labels

  4. Self-Monitoring

  5. The Importance of Physical Activity

  6. Diabetes Complications

  7. Medications

After completion of the class, participants are encoraged to join the Chronic Condition Support Group, in order to continue on the path to wellness. More information about that program can be found here.
