Dear Project Vida Health Center Patient,
Patient appointments in our clinics are limited and we must reserve them for those who are able to show up for their scheduled appointment times. Therefore, we have adopted a firm NO SHOW policy in order to minimize the number of failed appointments. This policy will be enforced as follows:
- Cancellations must be made at least 24 hours prior to your scheduled appointment time. Failure to do so will be considered a NO SHOW appointment.
- A NO SHOW fee of $10.00 will be added to your current balance, if any, or added to current charges for the same day services provided.
- After three NO SHOW appointments within a 6 month period, you will not be allowed to schedule an appointment for 6 months. You can be seen for emergency care only, and/or on a walk‑in basis. This does not guarantee you will be seen, scheduled appointments and emergencies will take priority.
- Patients non‑compliant with the NO SHOW policy will be referred to another Health Center facility for treatment.
