Once you complete a form, click Submit, and your information will be securely sent to our database. Our team will review your submission and contact you with any next steps or instructions.

REGISTRATION FOR UNINSURED/SLIDING SCALE

PLEASE UPLOAD the following information to register for services:

  • TX Driver's License or any current picture ID of patient and applicant *
  • Proof of address (i.e. a utility bill)
  • Last 2 current check stubs or letter verifying monthly income for patient and applicant
  • Birth certificates: for dependents 18 and under
  • Insurance cards: Medicare, Medicaid, CHIP, or private insurance for all applicants
  • Governmental assistance: Supplemental Nutrition Assistance Program (SNAP), RSDI
