Our Diabetes Care Program seeks to support the retention and continuum care for patients with diabetes type II by providing care follow-up, education, and referrals to needed social services, in order to reduce patients’ health disparities and address their non-medical drivers of health. Chronic Care Coordinators (CCCs) provide education, awareness, and the necessary tools for adequate self-management and the prevention of complications.

DIABETES SELF-MANAGEMENT EDUCATION (DSMEP)

Accredited by the Association of Diabetes Care & Education Specialists (ADCES)since 2012.

This program equips patients with the knowledge, skills, and support necessary to effectively manage type 2 diabetes. A free glucose meter is provided to participants who complete the entire course, which consists of 8 sessions.
