Project Vida Health Center's (PVHC's) Prevention & Chronic Care Program is the founding pillar of PVHC's Wellness & Prevention initiatives. For over 25 years, the focus of Wellness & Prevention has been to offer outreach and coordinated health promotion strategies for chronic care management.
Wellness & Prevention is staffed at all levels, from administration to education, by Promotoras/es (Community Health Workers). These trained and certified staff members provide classes in Pre-Diabetes Prevention, Diabetes Care, Managing Hyperlipidemia, Smoking Cessation, and Sexual & Reproductive Health Care. In addition to patients suffering from or at high risk for chronic illness, all patients benefit from comprehensive health education and wellness programs.
