Project Vida Health Center’s Smoking Cessation program supports patient retention and a continuum of care for adults willing to quit smoking. The program offers care follow-up, education, and referrals as needed to social services, all of which help reduce patients’ health disparities and address their non-medical drivers of health.
Smoking Cessation is provided by Chronic Care Coordinators certified by the American Lung Association “Freedom from Smoking®” program.
The Smoking Cessation Program is a structured, evidence-based cessation program designed to help adults quit smoking. It utilizes a combination of pharmacological and psychological techniques, including nicotine replacement therapy (subject to availability).
The program includes eight weekly sessions, led by trained facilitators, and includes participant workbooks and relaxation exercises.
This program is open to PVHC adult patients willing to quit smoking, regardless of whether or not they have health insurance, and is offered free of charge.
