The Hyperlipidemia/Hypertension Care Program supports the retention and continuum care of patients with high levels of cholesterol, high blood pressure, and high levels of LDL-cholesterol and/or triglycerides. The program provides care follow-up, education, and referrals to needed social services. This follow-up optimizes patient care, reduces health disparities, and addresses their non-medical drivers of health. The Chronic Care Coordinators listed below help ensure coordination of care.

ELIGIBILITY

Eligibility for the Hypertension Care Program includes patients of Project Vida Health Center at risk of developing hypertension from these risk factors:

  • Blood pressure +130/90 

  • Total Cholesterol +240 

  • Triglycerides +200
