Project Vida Health Center's Wellness & Prevention Department, through the Chronic Care Coordination Program, supports the retention and continuum of care of patients at risk of developing chronic diseases, such as diabetes, by providing care follow-up, education, and referrals to needed social services, to reduce patients’ health disparities and address their non-medical drivers of health.
The Diabetes Prevention Program (DPP)
The Diabetes Prevention Program helps adults at high risk of developing Type 2 diabetes to reduce their risk of developing the condition by taking steps to improve overall health and well-being. DPP Care Coordinators are trained as Lifestyle Coaches and offer a lifestyle-change curriculum developed by the CDC called Prevent T2, based on a psychosocial approach to both weight reduction and increase in physical activity to improve quality of life and risk reduction of type 2 diabetes.
