Case Management for Children and Pregnant Women
Our Case Management program helps children and pregnant women connect with the medical, social, educational, housing, food, financial, and other support they need. The program is available to Medicaid recipients ages 0–20 who have a health condition and/or a high-risk pregnancy. Interested?
