Hypertension Care Program supports the retention and continuum care of patients with high blood pressure and those who are at risk of developing it by providing: care follow-up, education, a blood pressure monitor (only if diagnosed with hypertension), and referrals to needed social services, to reduce patients’ health disparities and address their non-medical drivers of health. Participants will receive care coordination and 5 one-to-one education interventions.

ELIGIBILITY
Patients of Project Vida Health Center diagnosed with hypertension 130/90, or at risk of developing hypertension, by the following conditions:

Diabetes
-
Hyperlipidemia
-
BMI 30+
-
65+ years
-
Family History of Hypertension
-
Stress and tobacco usage/ alcohol >2 drinks a day
